Work Environment

Located in Uniondale, NY, our onsite practice operates within the unique scope of concierge medicine. This model allows us to prioritize our patients' needs, offering timely and accessible healthcare through house calls and personalized visits. Your work schedule will typically run from 9:00 a.m. to 6:00 p.m. with occasional evening clinics extending until 8:00 p.m. Additionally, you may be required to attend weekend community outreach events or health fairs, integrating your expertise into broader community health initiatives. This role averages around 40 hours per week, providing a balanced work schedule.

Key Responsibilities

• Perform holistic health assessments and thorough screenings for individuals and families to identify health risks and prevent potential conditions.
• Diagnose and manage both acute and chronic medical conditions, implementing personalized treatment plans that include preventive health measures.
• Collaborate with local organizations and stakeholders to design and implement community health education programs that address specific health challenges.
• Provide culturally sensitive care while advocating for patient rights, ensuring that all patients have equitable access to necessary healthcare services.
• Facilitate referrals and coordinate follow-up care, guaranteeing a seamless continuum of care for patients post-treatment.
• Maintain up-to-date, detailed, and accurate medical records in compliance with HIPAA and other regulatory standards, utilizing Electronic Health Record (EHR) systems.
• Engage in community outreach events and health fairs, offering health screenings and resources to promote wellness within the community.

Qualifications

Compensation and Benefits
