Work Environment

Located in Hempstead, NY, our facility operates in a supportive and collaborative environment where team members can flourish. The work setting is primarily onsite, with the flexibility to conduct house calls that offer convenience and personalized care for patients in their own homes. The typical work schedule runs from 9:00 a.m. to 6:00 p.m., though some evenings until 8:00 p.m. and weekend participation in community outreach events may be required, promoting our commitment to community health engagement.

Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse patient population, ensuring all aspects of health are evaluated.
• Diagnose and manage both acute and chronic health conditions, working closely with patients to establish tailored care plans that include preventive care and health promotion strategies.
• Partner with local community organizations and stakeholders to design and implement health education programs that inform and empower residents.
• Provide culturally competent care, actively advocating for patient rights and increased access to healthcare services for underserved populations.
• Coordinate referrals and follow-up care effectively, ensuring continuity in the patient care journey and aiding in recovery and health stability.
• Maintain accurate, detailed medical records compliant with all regulatory requirements, enhancing the quality of our care and operational efficiency.

Qualifications

Compensation and Benefits

Happy Haven Health Center offers a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
