Work Environment

The Nurse Practitioner will primarily work onsite in a home care setting, providing direct care to patients in their homes. This role supports a flexible and dynamic work environment where no two days are the same. The standard work hours are from 9:00 a.m. to 6:00 p.m., with the possibility of additional evening shifts until 8:00 p.m. to accommodate patient needs. Weekend participation in community outreach and health fairs is expected occasionally, thus fostering a deeper connection with the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, identifying their unique health needs.
• Diagnose and manage both acute and chronic health conditions, delivering preventive care and health promotion strategies tailored to patient requirements.
• Collaborate with local organizations to design and implement impactful health education programs that promote wellness and disease prevention.
• Advocate for patients’ rights and equitable access to healthcare, ensuring culturally sensitive care is provided at all times.
• Coordinate referrals and ensure timely follow-up care to create a seamless healthcare experience for patients.
• Maintain accurate and confidential patient records, adhering to all regulatory standards and best practices.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license in New York.
• A Master's degree in nursing from an accredited institution.

Compensation and Benefits

This role offers a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
