Work Environment

This position is based onsite in Great Neck, New York, where you will be actively involved in delivering healthcare directly to patients in their homes. Your work will include conducting medical assessments, follow-up visits, and health consultations in a comfortable, familiar environment for our patients. Key Responsibilities

• Conduct thorough health assessments and screenings tailored to individuals’ medical histories and current health status.
• Diagnose and manage a range of acute and chronic conditions, implementing evidence-based solutions and preventive care strategies.
• Collaborate with community organizations to create and deliver health education programs that elevate community awareness about various health issues.
• Advocate for patients, ensuring they receive culturally sensitive care that respects their backgrounds and healthcare needs.

Qualifications

• Current certification as a Nurse Practitioner.
• An active state nursing license in New York.
• A Master's degree in Nursing from an accredited educational institution.
• A minimum of 3 years of clinical experience, preferably within community health or public health settings.

Compensation and Benefits

This role offers a competitive salary range of USD 180,000 to 200,000 per year, commensurate with your qualifications and experience.
