Full-Time Nurse Practitioner in Concierge Medicine

Position Overview

We are looking for a skilled and compassionate Nurse Practitioner (NP) to join our dynamic healthcare team at Prattville Family Care Center. This full-time role focuses on delivering exceptional outpatient care within the innovative setting of concierge medicine, where you’ll make house calls to critically assess and manage patients' health needs in their homes.

Work Environment

You will work onsite, primarily interacting with patients in their homes, ensuring tender, friendly, and attentive care with every visit. The environment is characterized by teamwork and collaboration, integrating various specialties to create a cohesive healthcare experience. You will operate under the supervision of experienced Cardiology professionals, enhancing your skills and knowledge in outpatient care.

Key Responsibilities

• Collaborate closely with a team of cardiologists and healthcare professionals to deliver seamless patient care.
• Conduct thorough evaluations and history-taking, managing chronic conditions such as hypertension and high cholesterol.
• Provide outpatient-only Electrophysiology services, addressing abnormal electrocardiograms, arrhythmias, and device management.
• Manage acute cardiac conditions, including coronary artery disease, congestive heart failure, and stroke prevention, ensuring timely interventions are made.
• Administer stress testing when necessary to assess cardiac responses and provide pertinent medical advice.
• Provide comprehensive patient education to empower individuals and families regarding their health management.
• Refer patients to specialists within the practice as needed, coordinating care to optimize patient outcomes.

Patient Population / Client Focus

In this role, you will primarily work with adult patients experiencing various cardiovascular issues and general health needs. Your patient population will encompass individuals with chronic conditions, acute cardiac incidents, and preventive health needs, promoting a holistic approach to care that values each individual’s journey and understanding.

Qualifications

• A Bachelor’s degree in nursing or a related field, followed by a graduate-level Nurse Practitioner program.
• Certification as a Nurse Practitioner, with an active and valid State license to practice in New York.
• Proficiency in Spanish is required to effectively communicate with our diverse patient population.
• Demonstrated ability in outpatient cardiology or a related healthcare area, along with the completion of an accredited program.

Preferred Experience

• Previous experience in cardiology settings, including both inpatient and outpatient environments.
• A background in managing patients with a range of cardiovascular diseases and other chronic conditions.
• Familiarity with Electrophysiology services and cardiac device management.

Compensation and Benefits

We offer a competitive annual salary ranging from $140,000 to $180,000, commensurate with experience and qualifications. Our comprehensive benefits package includes:
