Work Environment

This full-time position requires your presence onsite at our assisted living facility in Astoria, where you will work in a supportive team environment dedicated to improving patient care. Your work schedule will typically run from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs on weekends may also be required. Expect an average weekly commitment of approximately 40 hours, contributing significantly to the health and well-being of our residents.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, identifying potential health issues and facilitating early interventions.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care and health promotion strategies to enhance patient health.
• Collaborate effectively with community organizations and stakeholders to design and implement impactful health education programs that resonate with community needs.
