Work Environment

This full-time position is based onsite within an assisted living facility, where you will work closely with the senior population navigating various health issues. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with flexibility for evening clinics operating until 8:00 p.m. on occasion, as well as potential participation in community outreach events during weekends. This ensures a dynamic work environment where no two days are the same, and you will engage directly with both residents and community members.

Key Responsibilities

• Conduct thorough health assessments and screenings for residents, identifying both acute and chronic conditions.
• Diagnose, manage and provide ongoing care for medical conditions, emphasizing preventive care and health promotion strategies tailored to individual needs.
• Collaborate with local health organizations to develop targeted health education programs and initiatives that foster wellness.
• Advocate for patients’ rights and needs, ensuring that they receive culturally competent care and appropriate access to healthcare services.
• Maintain accurate medical records in compliance with applicable regulatory standards, ensuring confidentiality and integrity of patient information.
• Proactively engage in community outreach activities, including health fairs and informational sessions that promote health awareness and provide critical health screenings.
• Continuously integrate knowledge of local health trends and evidence-based practices into patient care protocols.

Patient Population / Client Focus

Your primary focus will be the senior residents of the assisted living facility, many of whom may have complex health needs related to chronic illnesses, mobility issues, and mental health conditions. You will also engage with their families, offering guidance and support as they navigate the healthcare system. This role requires a deep understanding of the unique challenges faced by the elderly and a commitment to advocating for their health and well-being.

Qualifications

• Current certification as a Nurse Practitioner with an active nursing license in the state of New York.
• A Master’s degree in Nursing from an accredited program.
• Minimum of three years of clinical experience as a Nurse Practitioner, ideally within community health or public health environments.
• Exceptional interpersonal and communication skills, enabling collaboration with diverse populations and healthcare professionals.

Compensation and Benefits
