Work Environment

This role is based onsite, allowing you to deliver direct patient care within the comfort of their homes. The schedule involves working from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Participation in community outreach events or health fairs during weekends is a valued aspect of our service delivery, requiring flexibility in your schedule. Key Responsibilities

In this role, you will take on a variety of responsibilities crucial to the health of the community:

• Conduct thorough health assessments and screenings, ensuring each individual and family receives personalized care tailored to their unique health concerns.
• Diagnose and manage a wide range of acute and chronic medical conditions, implementing preventive care strategies and health promotion activities.
• Develop and collaborate on health education programs in partnership with community organizations that address specific health needs and challenges faced by residents.
• Deliver culturally sensitive care, actively advocating for patients' rights and ensuring equitable access to healthcare services.
• Facilitate smooth transitions for patients requiring referrals or follow-up care, ensuring continuity in their healthcare journey.

Patient Population / Client Focus

As a Nurse Practitioner with Angelic Care, you will primarily serve vulnerable populations in the Franklin Square community, including families facing socio-economic challenges and individuals with limited access to healthcare resources. Your role will involve understanding and addressing social determinants of health that influence patient outcomes.

Qualifications

The ideal candidate will possess the following qualifications:

• Current certification as a Nurse Practitioner, with an active state nursing license in New York.
• A master's degree in nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, preferably in community health or public health settings, demonstrating a deep understanding of patient care delivery in diverse environments.

Compensation and Benefits
