Work Environment

This position is based onsite and will primarily involve visiting patients in their homes. You will work in a dynamic environment, collaborating with community organizations and health stakeholders to promote wellness and preventative health strategies. Your schedule will typically span from 9:00 a.m. to 6:00 p.m., with the possibility of extended hours for evening clinics and involvement in weekend outreach events focused on health promotion.

Key Responsibilities

• Conduct thorough health assessments and screenings for both individuals and families, customizing care plans based on specific needs and circumstances.
• Utilize clinical expertise to diagnose and manage a variety of acute and chronic medical conditions, ensuring comprehensive preventive care and health promotion strategies are in place.
• Work closely with community organizations to design and implement health education programs that address the unique challenges faced by our diverse patient population.
• Provide culturally competent and sensitive care, advocating for patients' rights, and ensuring their healthcare needs are met effectively and ethically.
• Coordinate patient referrals and follow-up care, ensuring seamless transitions and continuity in patient care journeys.
• Maintain accurate and compliant medical records using Electronic Health Records (EHR) systems, adhering to healthcare regulations.
• Engage in community outreach activities, conducting health fairs and educational seminars to foster awareness and provide necessary health screenings.
• Stay current with community health trends, advances in medical practice, and relevant public health policies to inform care decisions and programming.

Qualifications

• Master's degree in nursing from an accredited institution.
• Current certification as a Nurse Practitioner and an active nursing license in the state of New York.
• Minimum of three to five years of clinical experience, with a preference for backgrounds in community health or public health settings.
• Excellent communication and interpersonal skills that enable productive interactions with a diverse patient population and community partners.

Preferred Experience

While a minimum of three years of experience is required, we highly value candidates with experience in home care settings, community health outreach, or roles that involve coordination with multidisciplinary teams. Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 per year, commensurate with your experience and qualifications.
