Work Environment

This role is conducted onsite in our outpatient office, where you will work alongside a multi-disciplinary team of healthcare professionals. The work schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach and health fairs, especially on weekends, may be required as part of our commitment to health education and community engagement. Overall, expect an average workweek of approximately 40 hours.

Key Responsibilities

• Conduct thorough health assessments and screenings for diverse individuals and families, ensuring an accurate and holistic understanding of their health status.
• Diagnose and manage both acute and chronic medical conditions, integrating preventive care practices and health promotion strategies tailored to community needs.
• Collaborate with local organizations and stakeholders to design and implement innovative health education programs aimed at enhancing community awareness and engagement.
• Provide culturally competent care and serve as a strong advocate for patients’ rights, ensuring equitable access to healthcare services.
• Coordinate referrals and follow-up care, maintaining an effective continuity of care to promote better patient outcomes.
• Maintain detailed and compliant medical records, ensuring adherence to all regulatory standards and best practices in patient documentation.
• Engage actively in community outreach events and health fairs, providing health screenings and education to raise awareness about healthcare resources.
• Stay informed about emerging community health trends, incorporating evidence-based practices and relevant health policies into your work.

Patient Population / Client Focus

In this role, you will primarily serve a diverse patient population, including low-income families, individuals with chronic health issues, and those who may face barriers to accessing quality healthcare services. Your approach will focus on understanding their unique situations, addressing cultural sensitivities, and advocating for health equity.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in nursing from an accredited program is required to ensure a comprehensive understanding of advanced nursing practices.
• A minimum of 3 years of clinical experience, ideally within community health or public health settings, demonstrating a strong background in diverse patient care.

Preferred Experience

Compensation and Benefits
