Work Environment

This position is based onsite and involves home care visits, requiring a blend of clinical expertise and community engagement. Your typical work schedule will run from 9:00 a.m. to 6:00 p.m., with occasional extension to 8:00 p.m. for evening clinics, and some weekend participation in outreach events or health fairs might be needed. The average workweek encompasses approximately 40 hours, fostering a structured yet dynamic environment that prioritizes both patient care and community outreach.

Key Responsibilities

• Conduct thorough health assessments, including physical exams, health history evaluations, and screenings to identify health needs in the community.
• Formulate and implement appropriate management plans for both acute and chronic conditions while emphasizing preventive care and health promotion strategies.
• Work collaboratively with local health organizations and stakeholders to develop impactful health education initiatives that address community health disparities.
• Deliver care that is culturally sensitive and tailored to the diverse population you serve, advocating fiercely for patients' rights and equitable access to healthcare.
• Facilitate continuity of care by coordinating referrals and follow-up for patients, ensuring they receive comprehensive healthcare services.
• Maintain meticulous and accurate patient records in accordance with all regulatory standards, ensuring compliance and confidentiality at all times.
• Engage in community outreach through health fairs and other events, providing vital health screenings and educational resources to enhance community wellness.
• Stay informed about community health trends, incorporating evidence-based practices and updates on relevant health policies into daily operations.

Patient Population / Client Focus

The Nurse Practitioner in this role will work closely with a diverse patient population, including families, children, and vulnerable individuals who typically do not have easy access to healthcare services. Your efforts will contribute significantly to improving their health literacy, preventative care options, and overall well-being.

Qualifications

• Valid Nurse Practitioner certification is required.
• Must possess an active New York state nursing license.
• A Master’s degree in nursing from an accredited institution is mandatory.
• Strong interpersonal and communication skills are essential, particularly when working with diverse community sectors.
• A genuine commitment to social justice, health equity, and advocacy for underserved populations.

Preferred Experience

• A minimum of three years of clinical experience, preferably within community health or public health settings.

Compensation and Benefits

At Spectra Community Health, we offer a competitive annual salary ranging from $180,000 to $200,000, commensurate with experience and qualifications.
