Work Environment

This role is onsite, providing face-to-face interaction with patients during regular office hours while also encompassing house call services. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinics and weekend community outreach activities. An average workweek totals about 40 hours, but flexibility may be necessary to accommodate patient needs.

Key Responsibilities

The Nurse Practitioner will undertake a range of critical responsibilities:

• Conduct comprehensive health assessments, including crucial screenings and diagnostic evaluations for patients of all ages.
• Diagnose and manage both acute illnesses and chronic conditions, developing individualized care plans aligned with best practices in family health.
• Implement preventive care strategies, including routine check-ups and health education initiatives, to enhance community health literacy.
• Foster collaborative partnerships with local health organizations, schools, and community groups to amplify health outreach efforts.
• Advocate for patients by addressing barriers to healthcare access and ensuring culturally sensitive care.
• Maintain accurate and detailed medical records in line with regulatory compliance and ensure continuity in care through effective management of referrals.

Qualifications

Compensation and Benefits
