Work Environment

You will primarily work onsite at an assisted living facility, where teamwork is paramount. With a focus on community health, our clinic operates within a multidisciplinary framework, collaborating closely with social workers, nutritionists, and other healthcare professionals. Your typical work schedule will be from 9:00 a.m. to 6:00 p.m., with flexibility to accommodate occasional evening clinics and community outreach activities, including health fairs on weekends. Expect to engage with a variety of patients in a warm and supportive atmosphere.

Key Responsibilities

• Conduct thorough health assessments, including comprehensive physical examinations and screenings for a diverse patient population, which includes families residing in assisted living facilities.
• Diagnose and manage a wide range of acute and chronic conditions, ensuring to emphasize preventive care and health promotion strategies that align with patient needs.
• Develop and implement tailored health education programs in collaboration with local community organizations, aimed at promoting wellness and preventing disease.
• Advocate for patient rights and equitable access to healthcare services while providing culturally sensitive care, addressing the unique needs of patients from diverse backgrounds.
• Maintain meticulous medical records and documentation that comply with regulatory requirements, ensuring continuity and quality of care through proper coordination of referrals and follow-ups.
• Participate actively in community outreach initiatives to enhance public health awareness and provide necessary health screenings to underserved populations.
• Remain informed about current trends in public health, community-specific health issues, and best practices to continually improve the quality of care provided.

Patient Population / Client Focus

Your primary focus will be on adult and pediatric patients in an assisted living environment, where you'll address their diverse and complex healthcare needs. This includes caring for patients with chronic illnesses, as well as offering preventive health measures and education that empower them to take charge of their health and wellness.

Qualifications

• Current certification as a Nurse Practitioner and a valid state nursing license.
• A Bachelor’s degree in nursing is required; a Master’s degree from an accredited program is preferred.
• A strong foundation in family medicine, with knowledge of community and public health practices.

Preferred Experience

• 3-5 years of clinical experience in community health, public health settings, or primary care frameworks.
• Experience working in assisted living facilities or similar environments is highly desirable.
• Demonstrated ability to engage with diverse populations and a commitment to health equity and social justice.

Compensation and Benefits

This role offers a competitive salary range of $120,000 to $220,000 per year, based on your qualifications and experience.
