Work Environment

As a Family Nurse Practitioner at Lifeline Family Care Center, you will work in a collaborative onsite outpatient office setting, allowing for continuous communication with your colleagues and an accessible environment for patient care. Our facility is structured to support a family-centered approach to health, equipped with the latest technology including Electronic Medical Records (EMR) systems to streamline patient management. Your work schedule will be Monday, Wednesday, and Friday from 8:00 a.m. to 5:00 p.m., providing a balanced work-life integration with no on-call duties or weekend work.

Key Responsibilities

• Conduct comprehensive primary care services that include thorough health assessments and evaluations tailored to individual patient needs.
• Perform onsite health risk assessments to identify potential health issues and develop actionable intervention plans aimed at improving patient health.
• Deliver educational programs for patients on wellness, preventive measures, and healthy lifestyle choices to empower them in their healthcare journey.
• Support patients managing chronic conditions by creating treatment plans and providing ongoing monitoring for optimal disease management.
• Collaborate with the medical staff within the Marathon Health Center to ensure coordinated and high-quality care.
• Engage in wellness events and health fairs, actively promoting health education and awareness initiatives within the community.
• Foster a collaborative relationship within the center's team to ensure the delivery of quality care and improve health outcomes.

Qualifications

Compensation and Benefits
