Key Responsibilities

• Conduct thorough health assessments and physical examinations for residents, documenting findings and creating detailed health records.
• Order, perform, and interpret diagnostic tests, collaborating with laboratory services to inform effective diagnosis and treatment planning.
• Develop and implement comprehensive, patient-centered care plans in collaboration with a multidisciplinary team, factoring in each resident's unique health needs.
• Prescribe medications, manage treatment protocols, and monitor resident responses to therapies.
• Engage patients and their families in meaningful health education sessions, providing guidance on disease prevention, health promotion, and lifestyle modifications.
• Participate actively in team meetings and case conferences to discuss progress, treatment adjustments, and care plan improvements.
• Maintain accurate and timely documentation of patient encounters, ensuring compliance with state regulations and best practices.
• Utilize electronic health record (EHR) systems efficiently to track patient care and coordinate with other healthcare providers.
• Contribute to ongoing quality improvement initiatives aimed at enhancing patient outcomes and services.

Patient Population / Client Focus

This role focuses primarily on adult patients residing in the assisted living facility, which includes seniors with various chronic conditions and complex health needs. You will be at the forefront of addressing both their acute medical concerns and ongoing health management, focusing on preventive care and wellness strategies tailored to a geriatric population.

Qualifications

• A valid and unrestricted Nurse Practitioner license in the state of New York.
• A Bachelor’s Degree in Nursing, with a Master’s degree in Nursing preferred.
• A minimum of two years of clinical experience as a Family Nurse Practitioner, particularly in geriatrics or assisted living settings.
• Robust clinical assessment and diagnostic skills, showing proficiency in handling complex health issues faced by seniors.
• Excellent communication skills, able to provide guidance and support to both residents and their families.
• Strong organizational abilities, with demonstrated success in managing multiple patient care tasks efficiently.

Compensation and Benefits
