Key Responsibilities

• Conduct comprehensive assessments of patients to develop tailored treatment plans that address their cardiovascular health needs.
• Utilize electronic health record (EHR) systems to maintain meticulous documentation of patient history, treatment plans, and outcomes, ensuring compliance with regulatory standards.
• Promote cardiovascular health education, advising patients and their families on lifestyle modifications, medication adherence, and disease prevention strategies.
• Collaborate with multidisciplinary teams on care coordination, participating in patient case meetings and sharing insights to enhance treatment effectiveness.
• Manage chronic cardiovascular conditions, including hypertension, heart failure, and arrhythmias through monitoring and medication adjustments as necessary.
• Participate in follow-up care through routine check-ups, ensuring continuous assessment of treatment efficacy and making recommendations as needed.

Patient Population / Client Focus

In this role, you will primarily serve older adults and seniors residing in an Assisted Living Facility, many of whom may present with complex cardiovascular conditions requiring specialized attention. This population often faces unique challenges associated with aging, necessitating a deep understanding of geriatric care principles, patient-centered communication, and psychosocial considerations.

Qualifications

• A Master’s degree in Nursing from an accredited institution is required.
• A current Nurse Practitioner license in New York State.
• A minimum of 2 years of documented clinical experience in cardiology or a closely related field.
• Proficiency with electronic health record (EHR) systems and other relevant medical software.
• Strong commitment to ongoing professional development and evidence-based practice.

Preferred Experience

• Previous experience in an Assisted Living or geriatric care setting is highly desirable.
• Certification from a national cardiology-specific board is an asset.
