Work Environment

The Nurse Practitioner will perform their duties entirely onsite, primarily visiting patients in their homes. Key Responsibilities

• Conduct thorough assessments, including history-taking and physical examinations, to diagnose and treat a variety of cardiology-related conditions such as hypertension, arrhythmias, and heart failure.
• Develop and implement comprehensive, personalized treatment plans tailored to each patient's medical history and health goals, involving a multidisciplinary approach with the healthcare team.
• Offer expert guidance on preventative care measures, including lifestyle changes, diet modifications, and medication management to enhance heart health.
• Monitor ongoing patient progress by reviewing diagnostic tests, interpreting results, and making necessary adjustments to treatment plans to ensure optimal patient outcomes.
• Provide education to patients and their families about cardiovascular conditions, addressing any questions or concerns they may have regarding their diagnoses and care pathways.
• Work collaboratively with cardiologists, primary care providers, and support staff to coordinate care effectively and facilitate referrals to specialty services when necessary.
• Stay updated on advancements in cardiology practices and integrate this knowledge into patient care, ensuring adherence to the latest treatment protocols and guidelines.

Patient Population / Client Focus

Qualifications

• A Master of Science in Nursing (MSN) degree with a focus on cardiology or completion of a Family Nurse Practitioner program that includes advanced cardiology coursework.
• Possession of the Cardiology Certified Nurse Practitioner (CCNP) credential from the Cardiology Nursing Certification Board (CNBC) is required.
• A minimum of 2 years of relevant experience in cardiology, primary care, or a related field as a Nurse Practitioner is essential.

Compensation and Benefits
