Key Responsibilities

• Conduct comprehensive primary care assessments, including initial evaluations and ongoing monitoring of patient health.
• Develop and implement individualized treatment plans based on clinical evaluations and diagnostic results.
• Perform routine physical examinations and utilize diagnostic equipment to assess cardiovascular health.
• Order and interpret diagnostic tests such as EKGs, echocardiograms, and blood tests, ensuring timely communication of results to patients.
• Educate patients and their families on cardiovascular health management, including lifestyle modifications and medication adherence.
• Document patient medical histories accurately and maintain thorough and organized medical records.
• Provide appropriate referrals to specialists and coordinate patient care to ensure comprehensive treatment pathways.
• Collaborate closely with physicians and other health professionals to advocate for patients and enhance care delivery.

Qualifications

• Current certification as a Nurse Practitioner.
