Work Environment

This full-time position is primarily onsite, where you will engage directly with patients in a range of settings. From private residences to skilled nursing facilities, your workplace will adapt to the needs of the patients we serve. Key Responsibilities

• Conduct comprehensive health assessments and screenings that evaluate both acute and chronic medical conditions, ensuring a thorough understanding of each patient’s health needs.
• Coordinate referrals and follow-up care, facilitating seamless transitions and continuity in patient care.
• Participate in community health outreach initiatives and health fairs, providing valuable health screenings and educational resources to enhance community health awareness.
• Maintain accurate medical records in accordance with regulatory standards, ensuring that documentation reflects comprehensive patient care and maintains confidentiality.
• Deliver culturally sensitive care, advocating for patient rights and equitable access to healthcare services across diverse population groups.
• Collaborate with community organizations to design and implement health education programs that address prevalent local health issues.
• Stay informed on the latest community health trends and evidence-based best practices to effectively manage care and inform patient education.

Patient Population / Client Focus

Qualifications

• A Master’s degree in nursing from an accredited program.
• An active state nursing license, with current certification as a Nurse Practitioner.
• A minimum of 3 years of clinical experience, ideally within community health or public health settings.
• Exceptional interpersonal and communication skills, enabling you to effectively connect with patients from various backgrounds.

Preferred Experience

Compensation and Benefits

We offer a competitive annual salary between USD 150,000 – 160,000 , in addition to performance-based bonuses. Our comprehensive compensation package includes:
