Key Responsibilities

• Conduct thorough health assessments and screenings for residents, focusing on cardiac health and related conditions.
• Diagnose and manage both acute and chronic cardiovascular conditions, providing medication management, lifestyle counseling, and preventive care.
• Coordinate referrals to specialists and ensure comprehensive follow-up care to support continuity in patient management.
• Document all clinical encounters and maintain meticulous medical records, adhering to regulatory standards and internal policies.
• Advocate for patients’ rights, ensuring they receive culturally sensitive care and have access to necessary resources.
• Engage in community outreach initiatives and health fairs to promote heart health awareness and provide essential health screenings to the public.

Patient Population / Client Focus

You will primarily work with elderly residents in the assisted living facility, many of whom have complex medical histories involving cardiovascular issues. Qualifications

• A Master’s degree in nursing from an accredited institution.
• An active state nursing license in New York.
• Current Nurse Practitioner certification.

Compensation and Benefits
