Full-Time Nurse Practitioner for Cardiology at Jewish Community Health Center

Position Overview

The Jewish Community Health Center is seeking a dedicated and skilled Nurse Practitioner (NP) to join our multidisciplinary team in Islip, New York. This full-time position is ideal for a compassionate healthcare provider who is eager to work in an outpatient setting, specifically within the Center for Advanced Cardiac Care (CACC). The NP will collaborate closely with a team of Cardiology experts under the direction of the Chief Nurse Practitioner and Medical Director, actively participating in the treatment and management of patients with cardiovascular conditions.

Work Environment

This role is based onsite in our Islip facility, focusing on concierge medicine and house calls. The NP will engage directly with patients in a comfortable and supportive atmosphere, where the emphasis is on personalized care that enhances patient satisfaction and promotes positive health outcomes. Working hours are set from Monday to Friday, 9 AM to 5 PM, ensuring a work-life balance free from night shifts, weekend obligations, or on-call duties.

Key Responsibilities

• Collaborate with several Cardiologists to support and manage a diverse patient caseload.
• Conduct thorough patient histories and physical examinations to inform treatment plans.
• Diagnose and manage chronic conditions including hypertension and high cholesterol.
• Provide specialized outpatient Electrophysiology services, assessing and treating patients with abnormal electrocardiograms, arrhythmias, and cardiac devices.
• Manage acute cardiovascular conditions such as Coronary Artery Disease, Congestive Heart Failure, and Peripheral Artery Disease, guiding patients through treatment protocols.
• Perform stress testing when necessary, leveraging advanced diagnostic tools to evaluate cardiac function.
• Refer patients to appropriate specialists within our group as required, fostering a seamless continuum of care.
• Offer education and support to patients and their families to promote understanding and adherence to treatment plans.
• Work closely with a diverse multi-disciplinary care team, providing comprehensive patient care that addresses both medical and emotional needs.
• Document patient interactions and treatment plans meticulously in our electronic health record (EHR) system to ensure continuity of care.

From those experiencing the complexities of multiple comorbidities to patients requiring specialized cardiac interventions, your role will be pivotal in managing their health and enhancing their quality of life. Qualifications

• Completion of a Bachelor’s degree in Nursing or a related field, followed by an accredited Nurse Practitioner program.
• Master’s or Doctoral degree in Nursing preferred.
• Possession of current state licensure as a Nurse Practitioner in New York.
• Certification as an Adult or Family Nurse Practitioner through a nationally recognized board.
• Strong foundational knowledge and competency in a variety of outpatient care practices.

Preferred Experience

While all applicants are encouraged to apply, candidates with prior experience in outpatient cardiology settings, particularly those with backgrounds in electrophysiology or arrhythmia management, will be given preference. Both seasoned practitioners and new graduates with relevant experience are welcome to apply.

Compensation and Benefits
