We are looking for a dedicated and skilled Nurse Practitioner (NP) specializing in Cardiology to join our team at Primary Care Center Inc. in Islip, New York. This full-time opportunity allows you to work in an onsite capacity at our Assisted Living Facility, where you will play a crucial role in providing high-quality outpatient cardiac care.

Work Environment

This position is based in Islip, NY, at our Assisted Living Facility, designed to support a diverse resident population. Key Responsibilities

In this role, you will have the opportunity to:

• Collaborate with cardiologists and a multidisciplinary team to deliver outpatient care to patients with cardiovascular conditions.
• Conduct comprehensive patient assessments, including taking medical histories and performing detailed physical exams to identify cardiovascular conditions and establish treatment plans.
• Manage chronic conditions such as Hypertension and High Cholesterol, making clinical decisions with significant autonomy under the guidance of the Medical Director.
• Provide specialized outpatient Electrophysiology services for patients with arrhythmias, offer stress testing, and manage patients with acute cardiac conditions like Coronary Artery Disease and Congestive Heart Failure.
• Educate patients and their families on disease management and lifestyle changes to support cardiac health.
• Prescribe medications as necessary and ensure that all treatment plans comply with current clinical guidelines.
• Refer patients to specialists within our integrated care team as needed to ensure comprehensive care.

Our patient demographic includes a wide range of individuals experiencing various cardiovascular conditions, from chronic respiratory issues to advanced heart failure. You will have the opportunity to work with diverse populations, tailoring your approach to the specific needs of each patient while providing compassionate and effective care.

Qualifications

To qualify for this position, candidates must possess:

• A Bachelor’s degree in Nursing and a Master’s or Doctoral degree in Nursing from an accredited program.
• Current Nurse Practitioner certification relevant to Cardiology.
• A valid NP license to practice in New York State.

Preferred Experience

We welcome applications from both seasoned Nurse Practitioners as well as new graduates with a keen interest in Cardiology. Prior experience in an outpatient setting focusing on cardiac care or related acute care experience is highly desirable. Familiarity with tools such as EHR systems for patient management will be advantageous.

Compensation and Benefits

This role offers a competitive salary ranging from USD 120,000 to USD 160,000 per year, commensurate with experience. In addition to a substantial salary, you will benefit from:
