Key Responsibilities

• Collaborate with physicians and healthcare team members to develop patient care plans that span the continuum of care.
• Conduct thorough assessments by obtaining detailed patient medical histories, performing physical examinations, and integrating psycho-social evaluations to form comprehensive health statuses.
• Implement preventive screening procedures tailored to the specific age group, ensuring early detection of potential health issues.
• Utilize clinical reasoning to formulate differential diagnoses and effectively prioritize treatment plans based on patient needs.
• Create educational materials and conduct discussions with patients and their families to enhance their understanding of health conditions and treatment plans.
• Prescribe and manage pharmacologic treatments as well as other therapeutic interventions, ensuring compliance with health standards and protocols.

Patient Population / Client Focus

This role primarily focuses on the pediatric and adolescent populations found within the school setting, which presents unique challenges and opportunities for health promotion. You will be responsible for addressing common health issues, managing chronic conditions in young patients, and promoting overall wellness through preventative care and education tailored to their needs.

Qualifications

• A Master’s Degree in Nursing with specialization as a Nurse Practitioner (NP).
• Possession of a current and valid Registered Nurse (RN) license in the state of New York.
• A minimum of 3 to 4 years of clinical experience in a relevant healthcare setting, preferably in primary care or internal medicine.
• Strong understanding of medical technology, including proficiency in electronic health records (EHR) systems.
• Excellent communication skills, with a strong focus on patient education and advocacy.
• Ability to diagnose and treat a diverse range of medical conditions, while developing and implementing effective treatment plans.

Preferred Experience

• Experience working in a school health setting or with a pediatric population is highly desirable.
