Work Environment

The successful candidate will work onsite in a school health facility located in Babylon, New York. Key Responsibilities

• Conduct thorough patient assessments through detailed interviews, comprehensive physical examinations, and a review of medical histories.
• Collaborate effectively with physicians and other healthcare professionals to develop, implement, and monitor individualized patient management plans.
• Engage with patients and their families to provide education about health conditions, medications, and preventative care strategies.
• Utilize electronic health record (EHR) systems to document patient interactions and ensure accurate information flow within the healthcare team.
• Participate in regular team meetings to discuss patient care strategies and community health initiatives.
• Develop and facilitate health screenings, immunization programs, and other preventive health measures in collaboration with school staff.
• Continuously evaluate and adjust care plans based on patient progress and feedback.

Patient Population / Client Focus

This position focuses on a diverse population of students within the Babylon school system, addressing a wide range of health concerns that may impact their educational experience. You will work closely with children and adolescents, providing compassionate healthcare while also guiding parents and guardians in understanding their health needs.

Qualifications

• A Bachelor’s Degree in Nursing or a related field from an accredited program.
• A current licensure as a Nurse Practitioner or Physician Assistant in the state of New York.
• Certification in the field of Internal Medicine is preferred.
• Completion of the Physician Assistant National Certifying Examination (PANCE) is required for PAs.
• Strong communication skills, both verbal and written.
• Ability to work effectively as part of a team and adapt to a dynamic work environment.

Preferred Experience

Compensation and Benefits
