We are seeking an experienced and compassionate Internal Medicine Nurse Practitioner or Physician Assistant to join our dedicated home care team in Uniondale, New York. Work Environment

This position is based onsite, primarily in patients' homes throughout the Uniondale area. You will be part of a multi-disciplinary team that includes nurses, social workers, and administrative staff dedicated to coordinating patient care. Key Responsibilities

• Perform comprehensive assessments of patients’ health status through detailed medical history interviews and physical examinations.
• Develop, implement, and manage individualized patient care plans in collaboration with our team of physicians and healthcare providers.
• Monitor and evaluate patient progress, making adjustments to care plans as necessary to address short-term and long-term health issues.
• Provide patient education, discussing the management of chronic conditions, medications, and lifestyle changes to promote healthier living.
• Document patient interactions and maintain accurate electronic health records to ensure continuity of care.
• Participate in team meetings and case conferences, sharing insights and recommendations for improving patient care.

Patient Population / Client Focus

This role is focused on providing care to adult and geriatric patients with a wide range of medical conditions, particularly those managing chronic illnesses such as diabetes, hypertension, and heart disease. You will work closely with patients and their families, establishing trust and rapport to support their healthcare journey.

Qualifications

• A Bachelor’s Degree in Nursing or a related field from an accredited program.
• Must have a current Nurse Practitioner or Physician Assistant license in the state of New York.
• Certification from the National Commission on Certification of Physician Assistants (NCCPA) or American Nurses Credentialing Center (ANCC) is preferred.

Preferred Experience

• A minimum of 2 years of clinical experience in an ambulatory care or home healthcare setting is highly desired.
• Experience in internal medicine, geriatrics, or chronic illness management, demonstrating stability and effectiveness in patient care delivery.

Compensation and Benefits
