Work Environment

This position operates on-site within a home care setting, where you will work alongside a collaborative care team that includes physicians, fellow nurse practitioners, physician assistants, and a support staff comprising medical assistants, registered nurses, and dedicated medical scribes. Together, we partner with a network of elite specialists and hospitals, ensuring that our patients receive comprehensive and coordinated care tailored to their unique needs.

Key Responsibilities

• Deliver top-notch primary care through independent patient evaluations and, when necessary, collaboration with physicians.
• Conduct thorough office visits addressing both routine check-ups and acute health concerns, ensuring accurate and detailed documentation.
• Administer annual wellness visits and health risk assessments, adopting a holistic approach to health.
• Coordinate care effectively with other healthcare providers, specialists, testing facilities, and community agencies, ensuring seamless transitions for our patients.
• Lead initiatives in population health, such as ensuring adherence to evidenced-based screening protocols to improve overall health outcomes.
• Actively participate in phone triage and community outreach to better connect with and educate patients regarding their care plans.
• Engage in promotional activities to enhance community awareness of our services and the importance of primary care.
• Perform home visits when required, ensuring accessibility to care for all patients.
• Report directly to the Medical Director and coordinate closely with operational leadership to align patient care strategies with organizational goals.

Patient Population / Client Focus

This role will primarily focus on providing personalized primary care to older adults on Medicare. Our innovative, community-centered model seeks to keep these patients active and healthy, enabling them to live fulfilling lives. By developing meaningful relationships with each patient, you will foster trust and a better understanding of their unique health goals and challenges.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and an active DEA license.
• A background in primary care settings such as internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and health risk assessments is essential.
• A genuine passion for teamwork and the desire to collaborate across various healthcare disciplines.

• Bilingual proficiency is a plus and may enhance patient communication.

Preferred Experience

• Experience working in a home care or community health setting, emphasizing a geriatric patient population.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 180,000 to 200,000, commensurate with experience and expertise. Additional benefits include:
