Work Environment

This role entails working onsite at our medical clinic, which specializes in concierge medicine and house calls. Our supportive environment is designed to facilitate teamwork and enhance patient care. The core operating hours for this position are Monday through Friday, from 9:00 AM to 5:00 PM. Additionally, you may occasionally conduct home visits, allowing you to provide comfort and care in the familiar surroundings of your patients.

Key Responsibilities

• Deliver comprehensive primary care services, including office visits for both routine and acute health issues.
• Administer annual wellness visits and health risk assessments, ensuring thorough documentation that accounts for the holistic health of patients.
• Coordinate care with a diverse team of healthcare providers, specialists, and agencies to streamline the patient's healthcare journey.
• Lead initiatives in population health management, ensuring adherence to preventive measures, such as breast cancer screenings for eligible female patients every two years.
• Engage actively in phone triage and outreach efforts to maintain open communication between patients and the care team.
• Educate patients on their health conditions, treatment options, and care plans, empowering them to take an active role in their wellbeing.
• Participate in promotional activities that foster community engagement and strengthen patient relationships.
• Document all clinical activities accurately, and regularly report to the Medical Director while collaborating closely with operational leadership.
• Fulfill additional responsibilities as assigned, demonstrating flexibility and a commitment to the clinic's mission.

Patient Population / Client Focus

This position primarily serves adult patients on Medicare, focusing on older adults who may have unique healthcare needs. Qualifications

• A Master of Science in Nursing, a Doctor of Nursing Practice degree, or a completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through either AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Prior experience in primary care settings, such as internal medicine, geriatrics, or family medicine.
• A thorough understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments.

Compensation and Benefits
