This role is based onsite at our hospital facility, where you will work directly with patients in a clinical setting. Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse population, identifying both immediate needs and long-term health goals.
• Utilize advanced clinical skills to diagnose and manage both acute and chronic medical conditions, developing personalized care plans that include preventive measures and health promotion activities.
• Collaborate with local organizations and community stakeholders to design and implement health education programs that address specific public health issues.
• Provide culturally sensitive care to all patients, ensuring their rights and access to services are duly respected and advocated for.
• Coordinate referrals and follow-up care with a focus on continuity of care and positive patient outcomes.
• Maintain comprehensive and accurate medical records using our electronic health record (EHR) system, adhering to all local and federal compliance standards.
• Engage actively in community outreach initiatives and health fairs, offering screenings and education to enhance public health awareness.
• Stay informed on current health trends, evidence-based practices, and relevant legislative policies that may impact community health.

Patient Population / Client Focus

You will work primarily with a diverse patient base, including individuals and families from varying socioeconomic backgrounds and health statuses. Your role will involve engaging with patients across all age groups, providing tailored care that respects and responds to their unique cultural and personal needs.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited institution.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A valid, unrestricted Nurse Practitioner (APRN) license in the State of New York.
• Possession of a valid DEA# and NPI#.
• Strong clinical assessment skills, with an emphasis on diagnostic capabilities.
• Exceptional communication and interpersonal skills to interact effectively with patients and team members.

Preferred Experience

• A minimum of 2 years of clinical experience in primary care or family medicine.
• Previous experience in community health or public health settings is highly desirable.
• Familiarity with electronic health records (EHR) systems.
• Demonstrated ability to work independently and as part of a collaborative team.

Compensation and Benefits

The salary range for this position is USD 150,000 – 160,000 per year. In addition to competitive compensation, NextGen NP Solutions offers a sign-on bonus of up to $10,000, comprehensive medical and dental insurance, retirement plan options, and generous paid time off.
