Key Responsibilities

• Conduct comprehensive assessments of patients with acute and chronic cardiovascular conditions, including collecting detailed health histories and performing thorough physical examinations.
• Utilize diagnostic tools effectively by ordering and interpreting laboratory tests, imaging, and other diagnostic modalities to develop evidence-based treatment plans.
• Prescribe appropriate medications and treatments in accordance with clinical best practices and department protocols, ensuring patients understand their treatment regimens.
• Lead patient education sessions, addressing chronic disease management, lifestyle modifications, and preventative health measures tailored to individuals based on their cultural, psychological, and educational needs.
• Collaborate closely with cardiologists, nursing staff, and multidisciplinary teams to facilitate seamless transitions of care and enhance patient outcomes.
• Participate in regular quality assurance and improvement initiatives aimed at refining patient care strategies based on clinical evidence and patient feedback.
• Document all patient encounters accurately in the electronic health record (EHR), maintaining the highest standards for information management and compliance.

Patient Population / Client Focus

The Nurse Practitioner in Cardiology will primarily serve an adult patient demographic, including those with chronic heart diseases, arrhythmias, heart failure, and hypertension. Qualifications

• A Master’s Degree in Nursing from an accredited program.
• Current Nurse Practitioner certification and licensure in New York.

Compensation and Benefits
