Experienced Nurse Practitioner - Cardiology Care Team

Position Overview

Gundersen Health and Wellness Clinic, located in Bellmore, New York, is actively seeking a dedicated Nurse Practitioner (NP) to join our expanding team within the Center for Advanced Cardiac Care (CACC). This full-time position offers an exciting opportunity to play a vital role in the outpatient practice setting, under the mentorship of the Chief Nurse Practitioner and guided by the Medical Director of the CACC, where your clinical expertise will directly influence patient care within a specialized cardiology environment.

Work Environment

This role is primarily onsite, allowing for dynamic interactions with patients, families, and healthcare professionals. Key Responsibilities

The Nurse Practitioner in this position will have a variety of responsibilities critical to patient management:

• Conduct thorough patient interviews and perform comprehensive and targeted physical exams.
• Order, interpret, and analyze diagnostic tests to accurately diagnose and manage both chronic and acute cardiology-related health issues.
• Develop and implement individualized, goal-oriented care plans in collaboration with physicians and allied health professionals, ensuring they align with established clinical protocols.
• Monitor patient progress and adjust treatment plans as necessary, following evidence-based guidelines and collaborating with the care team on complex cases.
• Provide education and counseling to patients and their caregivers regarding various disease processes and self-management strategies to enhance their understanding and compliance.
• Facilitate hospital admissions when needed, coordinating closely with the inpatient teams for safe transitions of care and discharge planning.
• Utilize electronic medical records (EMR) proficiently to document all patient interactions, treatment plans, and follow-up care.
• Engage in research protocols aimed at improving care practices and outcomes, incorporating findings into everyday clinical activities.

Patient Population / Client Focus

In this role, you will focus on a diverse patient population, including individuals with advanced heart failure, pulmonary arterial hypertension, and patients post-heart transplant. Your expertise will be invaluable in managing outpatient intravenous and inhaled medication support as well as mechanical circulatory support therapies, contributing to enhanced patient outcomes through quality care.

Qualifications

Candidates should possess the following qualifications to be considered:

• A Master's degree in Nursing as a Nurse Practitioner, specifically trained in adult care.
• Current licensing as a Registered Professional Nurse and Nurse Practitioner in New York State.
• NP Board certification, along with current certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency in electronic medical record systems and standard office software including MS Word, MS PowerPoint, and MS Excel.

Preferred Experience

While the minimum requirement is two years of clinical experience, preference will be given to candidates with:

• Experience working in a hospital setting, particularly in cardiology.
• Specific patient care experience with advanced heart failure patients and those undergoing advanced therapies.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $220,000 annually, commensurate with experience and qualifications. Our comprehensive benefits package includes:
