The Gundersen Health and Wellness Clinic is actively seeking a skilled and dedicated Nurse Practitioner (NP) to join our specialized team at the Center for Advanced Cardiac Care (CACC) in Baldwin, New York. This full-time, onsite position focuses on delivering high-quality care to patients in need of cardiovascular services, with an emphasis on both chronic and acute conditions. The NP will operate under the leadership of the Chief Nurse Practitioner and in collaboration with the CACC Medical Director, ensuring that all care provided aligns with the highest standards of clinical excellence.

Key Responsibilities

• Conduct thorough patient assessments, including obtaining detailed histories and performing comprehensive physical examinations.
• Order and evaluate diagnostic tests to effectively diagnose and manage a variety of cardiac conditions.
• Develop and implement individualized care plans that focus on health maintenance, disease prevention, and management of chronic and acute issues.
• Document all patient interactions and clinical findings accurately within our electronic medical records (EMR) system to support continuity of care.
• Engage in ongoing collaboration with medical and nursing staff to reassess and adapt treatment strategies based on patient progress and emerging health needs.
• Educate patients and their caregivers on health management strategies and disease processes, ensuring they understand how to navigate care outside the hospital environment.
• Coordinate with multidisciplinary teams to facilitate hospital admissions and ensure safe discharges, maintaining communication with primary care providers.
• Participate in research initiatives, utilizing the latest findings to enhance clinical practice and improve patient outcomes.
• Contribute to maintaining a clean, safe, and supportive clinic environment focused on patient care excellence.
• Complete additional responsibilities as assigned to support overall team objectives.

Patient Population / Client Focus

The Nurse Practitioner at Gundersen Health and Wellness Clinic will primarily serve adults with a focus on those requiring specialized cardiac care. This includes patients managing chronic conditions such as advanced heart failure and pulmonary arterial hypertension, as well as those undergoing treatments for mechanical circulatory support and post-heart transplant care. Qualifications

• A Master’s degree in Nursing, with specialized training as a Nurse Practitioner focused on adult care is essential.
• Active New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Board certification as a Nurse Practitioner, ensuring adherence to industry standards.
• Current certifications in BCLS, ACLS, DEA, and NPI are required for practice.

Compensation and Benefits
