Position Overview

The Gundersen Health and Wellness Clinic is actively recruiting an experienced Nurse Practitioner (NP) to join our dynamic Center for Advanced Cardiac Care (CACC) based in Woodside, New York. In this full-time, onsite role, you will work closely with a multidisciplinary team to deliver exceptional care within an Assisted Living Facility. Reporting directly to the Chief Nurse Practitioner and collaborating with the CACC Medical Director, your contributions will be pivotal in enhancing patient care outcomes while fostering a teamwork-oriented environment.

Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed medical histories and performing comprehensive physical examinations to establish an effective diagnostic and treatment plan.
• Order, interpret, and communicate results from diagnostic tests, managing both acute and chronic conditions in collaboration with other healthcare providers.
• Develop individualized, goal-directed care plans that include health maintenance and appropriate educational resources for patients and their families.
• Document all patient encounters accurately in electronic medical records (EMR) to maintain continuity of care and ensure compliance with standard protocols.
• Facilitate care transitions by coordinating hospital admissions and working closely with inpatient teams for safe discharge planning.
• Educate patients and caregivers about disease processes, treatment options, and self-management strategies to empower them in their health journeys.
• Participate in research initiatives by reviewing and integrating findings into clinical practices, contributing to the advancement of cardiology care.
• Work collaboratively with a multidisciplinary team of specialists, including pharmacists, dietitians, and social workers, to coordinate patient care across various settings.
• Actively partake in team meetings to discuss patient care strategies and streamline communication with referring providers.

Patient Population / Client Focus

As a Nurse Practitioner in our cardiology department, you will primarily work with patients experiencing a range of cardiovascular issues, including advanced heart failure, pulmonary arterial hypertension, and those undergoing complex treatments such as mechanical circulatory support. Your role will involve providing both acute care and chronic disease management, ensuring that all care strategies are tailored to the unique needs of each patient.

Qualifications

• Master's degree in Nursing with a focus on adult care as a Nurse Practitioner.
• Current New York State licenses as a Registered Professional Nurse and Nurse Practitioner.
• Board certification as a Nurse Practitioner.
• Certifications in Basic Life Support (BCLS) and Advanced Cardiovascular Life Support (ACLS).
• DEA and NPI certifications are required.
• Proficient in using electronic medical record (EMR) systems and basic computer applications including MS Word, Excel, and PowerPoint.

Preferred Experience

• A minimum of two years of experience in a hospital setting, preferably in cardiology.
• Familiarity with managing patients in advanced heart failure and post-heart transplant care.
• Experience in coordinating therapies involving outpatient intravenous treatments or inhaled medication support.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 to 220,000 per year, commensurate with experience and qualifications.
