The Gundersen Health and Wellness Clinic is excited to invite applications for the role of Nurse Practitioner (NP) within our Center for Advanced Cardiac Care (CACC) outpatient facility located in Yonkers, New York. This full-time, onsite position is tailored for an experienced NP who is passionate about delivering exceptional patient-centered care in a dynamic cardiology environment. The NP will work under the guidance of the Chief Nurse Practitioner and the CACC Medical Director, contributing to a collaborative team dedicated to advancing cardiac health.

Key Responsibilities

As an NP within our esteemed Cardiology Clinic, your responsibilities will encompass the following:

• Conduct thorough patient histories and perform detailed physical examinations tailored to the needs of cardiovascular health.
• Order and interpret diagnostic tests, making informed decisions to diagnose and manage both acute and chronic cardiac conditions.
• Collaborate with physicians and peers to develop effective, individualized treatment plans that may involve health maintenance strategies and necessary medical interventions.
• Guide and educate patients and their caregivers regarding disease processes, treatment options, and post-discharge care plans.
• Facilitate hospital admissions, ensuring seamless coordination with inpatient teams during patient transitions and safe discharge processes.
• Work closely with allied health professionals to triage and address patient calls, while maintaining effective communication with referring providers.
• Document all patient encounters accurately in the EMR to ensure continuity of care and compliance with regulatory standards.
• Actively participate in research initiatives, applying findings to enhance clinical practice and contribute to the advancement of cardiac care.
• Maintain accountability for your nursing practice by adhering to established protocols and ongoing professional development.
• Participate in creating a safe and patient-friendly environment, emphasizing the highest standards of care.

Patient Population / Client Focus

The Cardiology Clinic primarily serves adult patients with a wide array of cardiac conditions. Our patient population includes individuals with chronic heart failure, pulmonary arterial hypertension, and those undergoing advanced therapies such as outpatient intravenous treatments, inhaled medication support, and mechanical circulatory support. The NP will thrive in an environment that demands both autonomy and collaboration, prioritizing holistic patient care and education to motivate patients on their journeys to better health.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master’s degree in Nursing, with NP training specifically in adult care.
• A current New York State Registered Professional Nurse and Nurse Practitioner license.
• NP Board certification.
• Current certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency with electronic medical record systems, as well as standard computer applications such as MS Office Suite (Word, Excel, PowerPoint).

Preferred Experience

While a minimum of two years of experience within a hospital setting is required, we highly value candidates who have:

• Direct experience with cardiology, specifically working with heart failure and post-heart transplant populations.
• Hands-on experience in managing patients under advanced cardiac therapies and support systems.

Compensation and Benefits

The position offers a competitive salary range of USD 120,000 – 220,000 annually, reflective of your experience and qualifications.
