Key Responsibilities

• Conduct thorough cardiac assessments and examinations, utilizing diagnostic tools such as EKGs, Holter monitors, and stress tests.
• Diagnose and manage various cardiovascular conditions, including coronary artery disease, heart failure, arrhythmias, and hypertension with a personalized approach.
• Collaborate with interventional cardiologists to develop customized treatment plans that address individual patient needs and goals.
• Implement patient-specific treatment plans that encompass medication management, lifestyle changes, and educational support.
• Foster collaborative relationships with cardiologists, nurses, and allied healthcare staff to facilitate the best possible patient outcomes.
• Build rapport with patients and their families, offering empathetic communication to enhance the treatment experience.

Patient Population / Client Focus

This role primarily focuses on the elderly resident population in an Assisted Living Facility. Many patients may face multiple comorbidities, requiring a holistic, multifaceted approach to their care. Understanding the unique challenges of this demographic will be essential in effectively diagnosing and treating complex cardiovascular issues.

Qualifications

• Master's degree in Nursing with a specialization in Nurse Practitioner studies from a recognized program.
• A valid and active Registered Nurse (RN) License in the state of New York.
• Current Basic Life Support (BLS) certification.
• Proficiency in electronic medical record systems is highly desirable.

Preferred Experience

Compensation and Benefits
