Work Environment

This role is full-time and primarily onsite, providing direct patient care in a home care setting tailored to meet the needs of our patients in their own living environments. Our team operates Monday through Friday, for a total of 40 hours per week, with rotating Saturday coverage to ensure comprehensive patient access. Key Responsibilities

• Conduct thorough evaluations by managing both the physical and psychological aspects of patient care, ensuring a holistic approach to treatment.
• Identify normal and abnormal findings during patient assessments and document findings accurately to inform treatment plans.
• Develop, implement, and continuously reassess individualized treatment plans based on patient needs, responses to treatment, and ongoing assessments.
• Monitor and evaluate patients' physical and emotional reactions to medications and interventions, making necessary adjustments to treatment protocols.
• Prescribe immunizations according to established guidelines and protocols to enhance patient health and preventive care measures.
• Foster effective communication with supervising physicians and collaborating healthcare providers to ensure a seamless continuum of care for each patient.
• Maintain an accurate Problem/Medication list at every encounter, ensuring that patient records are complete and current.
• Adhere to regulatory documentation requirements, ensuring accuracy, completeness, and timeliness in all patient records.

Patient Population / Client Focus

The Nurse Practitioner will focus primarily on patients with cardiovascular conditions, ranging from acute cases requiring immediate intervention to chronic management needs. Our patient demographic includes diverse populations, each bringing unique health backgrounds and requiring tailored care approaches to ensure optimal health outcomes.

Qualifications

• A Master's degree in Nursing with a specialization as a Nurse Practitioner from an accredited institution.
• A current Registered Nurse (RN) license in the State of New York.
• Certification in Basic Life Support (BLS).
• Strong interpersonal, communication, and organizational skills, demonstrating the ability to engage effectively with patients of diverse backgrounds.

Preferred Experience

• A minimum of 2 years of clinical experience as a Nurse Practitioner is required, with a strong preference for candidates with background experience in cardiology.
• Experience in home care settings will be considered an asset.

Compensation and Benefits

This position offers a competitive annual salary range of $150,000 to $160,000, depending on experience and qualifications.
