Work Environment

This position is onsite and involves a hybrid model of care that includes both in-clinic consultations and house calls, making it a unique opportunity for Nurse Practitioners who enjoy a versatile practice setting. Key Responsibilities

• Conduct thorough assessments of patients experiencing cardiac symptoms or managing chronic cardiac conditions to ensure a comprehensive understanding of each patient’s health status.
• Order, interpret, and explain diagnostic tests, including EKGs, echocardiograms, and stress tests, to ensure accurate diagnosis and effective treatment plans.
• Collaborate closely with cardiologists and an array of healthcare professionals to design and implement individualized patient care plans that encompass medication management, lifestyle modifications, and comprehensive patient education.
• Continuously monitor and evaluate the effectiveness of treatment modalities, proactively making necessary adjustments to care plans to optimize patient outcomes.
• Provide education and counseling to patients and their families, helping them understand cardiac conditions, treatment options, and strategies for effective management.
• Stay current with the latest advancements in cardiology, employing evidence-based practices and contributing to ongoing quality improvement initiatives within the center.

Patient Population / Client Focus

At Bright Touch Medical Center, you will work with a diverse population of patients with various cardiac conditions, from hypertension and congestive heart failure to complex arrhythmias. This role emphasizes a patient-centered care model, where you will not only focus on managing symptoms but also on improving overall quality of life through proactive health education and lifestyle adjustments.

Qualifications

• An active, unencumbered Nurse Practitioner licensure is required, free of any disciplinary actions.
• National certification from either the ANCC or AANP in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care is necessary, with the expectation of meeting state licensing requirements by the start date.
• Proficiency in completing the physical requirements of the job, including lifting up to 30 pounds and navigating various dwelling conditions to perform essential assessments.

Preferred Experience

While a minimum of two years of experience in clinical practice as a Nurse Practitioner is required, ideal candidates will have backgrounds in gerontology, cardiology, internal medicine, or endocrinology. Experience with patient education and personalized care planning is highly desired.

Compensation and Benefits
