Key Responsibilities

• Conduct thorough health assessments and screenings for patients, focusing on both acute and chronic cardiac conditions.
• Diagnose and manage various cardiovascular conditions, implementing preventive measures and health promotion activities.
• Collaborate with cardiologists and other healthcare providers to develop individualized treatment plans while participating in patient rounds.
• Document all patient interactions accurately and maintain up-to-date medical records in compliance with healthcare regulations.
• Lead community outreach initiatives, including health fairs and educational programs, to improve awareness of heart health among local populations.
• Utilize data from health trends and analytics to inform patient care strategies and public health initiatives.

Patient Population / Client Focus

Qualifications

• Current credential as a Nurse Practitioner.
• Active Nursing Licensure in the State of New York.
• Master’s degree in nursing from an accredited institution.

Compensation and Benefits

Heart Link Care Services offers a competitive salary ranging from $150,000 to $160,000 annually, commensurate with your experience and qualifications.
