Key Responsibilities

• Conduct detailed cardiovascular assessments and screenings, gathering patient histories and vital signs to evaluate heart health.
• Diagnose and manage both acute and chronic cardiovascular conditions, effectively utilizing diagnostic tests such as EKGs, echocardiograms, and stress tests to guide treatment decisions.
• Develop, implement, and adjust treatment plans based on patient progress and current best practices in cardiac care.
• Engage in patient education by counseling individuals and families on heart disease prevention, medication adherence, and lifestyle modifications.
• Maintain accurate, up-to-date medical records following regulatory standards and best practices, utilizing Electronic Health Records (EHR) systems for documenting patient interactions and treatment plans.
• Work collaboratively with multidisciplinary teams and community organizations to foster health promotion initiatives aimed at raising awareness of cardiovascular health issues.
• Stay informed on emerging trends and developments in cardiology and related healthcare policies to advocate for quality patient care.

Patient Population / Client Focus

This position focuses on a diverse patient population, primarily older adults with varying levels of cardiac health. You will be responsible for addressing the unique healthcare needs of residents in the Assisted Living Facility, many of whom may have complex medical histories and require personalized care approaches. The role emphasizes culturally sensitive care and advocates for equitable access to necessary cardiology resources.

Qualifications

• Master's degree in Nursing from an accredited program, with a specialization in Cardiology preferred.
• Active state nursing license with certification in Cardiovascular Nursing or a related specialty is required.
• A minimum of 3 years of direct clinical experience as a Nurse Practitioner, specifically in cardiology or cardiovascular health settings.

Preferred Experience

• Experience working within community health settings or Assisted Living Facilities.

Compensation and Benefits
