Work Environment

This position is based onsite at our assisted living facility, where you will have the opportunity to work closely with a diverse patient population, including elderly individuals with complex cardiovascular conditions. Key Responsibilities

• Maintain precise and up-to-date medical records, ensuring all documentation complies with healthcare regulations and internal protocols.
• Collaborate effectively with fellow healthcare professionals to create and implement holistic care plans that address the physical and emotional needs of patients.
• Conduct routine wellness check-ups for patients, monitoring their treatment progress and making necessary adjustments in line with best practices.
• Prescribe and manage medications and therapies, adhering to established guidelines to optimize patient health.
• Conduct assessments and manage chronic cardiovascular conditions, such as hypertension, heart failure, and arrhythmias, in a timely manner.
• Participate in educational sessions and workshops to enhance the healthcare team's knowledge of current cardiology practices and innovations.

Patient Population / Client Focus

The patient population primarily consists of elderly individuals who often present with multiple comorbidities, including cardiovascular diseases. As a Nurse Practitioner specializing in Cardiology, you will play an essential role in monitoring and managing these complex health needs, ensuring that care is tailored to each patient's unique circumstances and improving their overall quality of life.

Qualifications

• Graduate from an accredited Master’s program in Nursing with a focus on Nurse Practitioner studies.
• Possession of a current Nurse Practitioner license valid in the state of New York.
• Demonstrated experience in a cardiology or cardiovascular-focused role with a minimum of two years in clinical practice.
• Strong interpersonal skills and the ability to work both independently and collaboratively in a team-based setting.

Preferred Experience

• Experience in an assisted living or long-term care facility is highly desirable, as it will enhance your ability to connect with our patient demographic.
• Familiarity with electronic health record (EHR) systems and integrated care management software.
• Additional certifications in cardiology or related fields will be considered an asset.

Compensation and Benefits

This full-time position offers a competitive yearly salary range of USD 150,000 to 160,000, commensurate with experience and qualifications.
