Work Environment

In this role, you will operate within the dynamic and flexible setting of concierge medicine, prioritizing convenience and accessibility for patients. The working environment is patient-focused, where you will spend the majority of your time visiting clients in their homes. Key Responsibilities

• Conduct thorough assessments of patients' cardiovascular health through in-person consultations, focusing on their individual health histories and current conditions.
• Develop and implement individualized treatment plans that account for the unique needs and preferences of each patient, and provide supportive education to help them make informed decisions.
• Perform initial examinations, including comprehensive physical exams, while utilizing advanced diagnostic tools and techniques to evaluate patients' conditions effectively.
• Manage and maintain essential medical equipment, ensuring all tools are functional and sterile for the highest level of care.
• Order, interpret, and communicate results from diagnostic tests, such as EKGs and lab work, while ensuring that patients understand their results and proposed next steps.
• Document accurate patient medical histories in electronic health records and make timely referrals to specialists when necessary based on evaluation findings.
• Provide follow-up care and support, regularly monitoring patients' progress and adjusting treatment plans as needed.

Patient Population / Client Focus

You will primarily serve patients with a variety of cardiovascular issues, including but not limited to hypertension, heart failure, and arrhythmias. The focus is on delivering service to individuals who may be underserved or have difficulty accessing traditional healthcare facilities. Your role will be crucial in managing chronic conditions, educating patients on lifestyle modifications, and preventing serious cardiac events through proactive care strategies.

Qualifications

• Current certification as a Nurse Practitioner with a specialty in cardiology.
• Active nursing license in the state of New York.
• A Master's degree in nursing from an accredited institution.
• A minimum of 2 years of clinical experience, ideally within community health or cardiology settings, to ensure competency in managing cardiac patients.

Preferred Experience

• Experience in concierge medicine or house call practices, providing a foundational understanding of patient-centered, outpatient care.
• Proficient in using electronic health record (EHR) systems and comfortable with remote monitoring technologies and telehealth tools.
