Work Environment

This position is based in Rye, New York, where our healthcare professionals utilize a concierge model to ensure that patients receive unparalleled attention and care. Our Nurse Practitioners operate onsite, engaging directly with patients in their homes or in our clinical setting.

Key Responsibilities

• Conduct detailed patient histories and perform comprehensive physical examinations to assess individual health needs.
• Order, perform, and interpret diagnostic tests, such as EKGs and blood work, in line with each patient’s concerns.
• Prescribe and manage pharmacological and non-pharmacological strategies tailored to patient-specific treatment plans.
• Maintain accurate and thorough records in patient charts, documenting treatment plans, progress notes, and relevant communication.
• Collaborate with supervising physicians on health maintenance protocols and ensure adherence to established care pathways.
• Engage regularly with patients and their families, effectively communicating essential information regarding treatment options and health status.
• Support cardiologists through hospital rounding and in-basket tasks, fostering a seamless care process across various settings.

Patient Population / Client Focus

The Nurse Practitioner will primarily focus on adult patients with various cardiovascular conditions, ensuring individualized care plans that prioritize optimal health outcomes. This role is integral to enhancing patient access to quality care through house calls and personalized follow-up services.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• Current Registered Nurse (RN) license in the State of New York.
• A minimum of 3 years of clinical experience, preferably in cardiology or a related field.
• Basic Life Support (BLS) certification is mandatory.
• Strong interpersonal and communication skills, with the ability to effectively engage diverse patient populations.

Preferred Experience

• Experience in a concierge or house call healthcare model is highly desirable.
• Familiarity with electronic health record (EHR) systems to facilitate documentation and communication.
• Proven ability to coordinate care among multidisciplinary teams and manage complex patient cases.

Compensation and Benefits

• The position offers a competitive salary ranging from USD 150,000 to 160,000 annually, commensurate with experience.
