Key Responsibilities

• Perform comprehensive health assessments on patients presenting with cardiac conditions, including conducting physical examinations and diagnostic evaluations.
• Develop and implement care plans tailored to individual patient needs, effectively managing both acute and chronic cardiac issues.
• Provide education and support to patients and their families regarding disease management, lifestyle modifications, and preventative care strategies.
• Actively participate in clinical rounds, collaborating with cardiologists and other specialists to optimize patient care and improve treatment protocols.
• Maintain thorough and accurate medical records in accordance with regulatory standards and organizational policies.
• Lead community health initiatives, conducting health screenings and educational workshops to enhance cardiovascular health awareness in the community.
• Utilize telehealth technologies to provide follow-up care and consultations, expanding access to quality healthcare for our patients.

Patient Population / Client Focus

As a Cardiology Nurse Practitioner, you will work with a diverse patient population, from young adults to the elderly, presenting a broad range of cardiac conditions. You will play a crucial role in addressing the specific needs of individuals with heart disease, hypertension, heart failure, and other cardiovascular issues, while also advocating for preventive healthcare measures to improve community health.

Qualifications

• Possession of a Nurse Practitioner credential and active State Nursing Licensure in New York.
• A Master’s degree in Nursing from an accredited program is required.
• A minimum of two (2) years of clinical experience in a Cardiology setting is essential, demonstrating a strong foundation of cardiac care knowledge.
• Excellent interpersonal and communication skills, essential for establishing rapport with patients and collaborating effectively with healthcare team members.

Preferred Experience

Compensation and Benefits
