The Nurse Practitioner will work onsite in a fast-paced outpatient setting. Key Responsibilities

• Conduct comprehensive health assessments through detailed physical examinations and health screenings for patients with cardiovascular conditions.
• Diagnose and manage both acute and chronic cardiology-related diseases such as hypertension, heart failure, and arrhythmias, administering preventive care and wellness promotion.
• Develop and implement tailored care plans while collaborating closely with our team of healthcare providers to ensure integrated and effective care.
• Engage with community organizations and local stakeholders to design, and promote health education initiatives, thus fostering community health and well-being.
• Advocate for patients' rights, ensuring access to essential healthcare services while providing culturally sensitive care tailored to the needs of diverse populations.
• Coordinate referrals and follow-up care seamlessly to ensure continuous patient support and recovery.
• Maintain meticulous medical records in accordance with regulatory requirements, utilizing electronic health record systems effectively to track patient care.
• Participate in local outreach events including health fairs and screening programs, representing Health Express Urgent Care and raising public awareness regarding cardiovascular health issues.
• Stay informed on the latest developments in cardiology and community health trends, applying evidence-based practices consistently in your work.

Patient Population / Client Focus

As a Nurse Practitioner specializing in Cardiology, you will primarily work with patients who have varying degrees of cardiac health issues, including but not limited to adults with a history of cardiovascular diseases and those seeking preventive care measures. Your role will focus on empowering patients through education and engagement in their health journeys, thereby managing their conditions effectively.

Qualifications

• A Master's Degree in Nursing from an accredited academic institution.
• An active nursing license in the state of New York.
• Current certification as a Nurse Practitioner, with a specialty in cardiorespiratory health preferred.
• A strong background in cardiology with at least three years of relevant clinical experience, particularly in outpatient settings.
• Excellent interpersonal and communication skills, allowing for effective interactions with a diverse patient demographic.
• A commitment to social justice, health equity, and a passion for advocating for underserved communities.

Preferred Experience

• Experience in integrated care models and familiarity with electronic health records (EHR) systems.

Compensation and Benefits
