Experienced Nurse Practitioner - Internal Medicine at CareMed

Position Overview

We are seeking a dedicated and experienced Nurse Practitioner specializing in Internal Medicine to join our dynamic healthcare team at CareMed Primary and Urgent Care, located in New Rochelle, New York. This full-time position offers a competitive salary ranging from USD 150,000 to 160,000 annually. As a Nurse Practitioner, you will play a vital role in providing high-quality patient care, coordinating treatments, and ensuring the overall health and well-being of residents at our Assisted Living Facility.

Work Environment

This position is onsite at our Assisted Living Facility, which features a welcoming and collaborative atmosphere. Our facility is equipped with advanced medical technology, and we utilize an efficient electronic health records (EHR) system to enhance patient care and streamline communication among healthcare providers. You will work closely with a team of physicians, nurses, and support staff to deliver integrated medical services.

Key Responsibilities

• Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans for residents with diverse medical histories.
• Conduct comprehensive health assessments that include obtaining detailed medical, physical, and psycho-social histories, as well as performing thorough physical examinations.
• Utilize preventive screening procedures based on patients' age and medical history to identify potential health risks and intervene proactively.
• Formulate and confirm differential diagnoses through careful evaluation of patient history, physical exams, and clinical findings, ensuring accurate treatment paths.
• Educate patients and their families about health management, prescribed treatments, and lifestyle modifications to enhance overall wellness.
• Prescribe and administer pharmacologic and non-pharmacologic therapies in accordance with established protocols and best practices, ensuring optimal patient care.

Patient Population / Client Focus

In this role, you will primarily serve elderly residents who require ongoing medical care and support within the Assisted Living Facility. Your expertise will be instrumental in managing a range of chronic conditions, as well as addressing acute health issues. You will develop strong, trusting relationships with patients and their families, ensuring they feel heard and cared for throughout their healthcare journey.

Qualifications

• A Master's Degree in Nursing with a specialization as a Nurse Practitioner.
• Active Registered Nurse (RN) license in New York State.
• A minimum of 3-4 years of relevant clinical experience, preferably in a primary care or geriatrics setting.
• Proficiency in utilizing medical technology and electronic health records (EHR) systems to enhance patient care delivery.
• Strong communication and interpersonal skills, with the ability to effectively educate patients and advocate for their healthcare needs.
• Demonstrated ability to diagnose and treat a wide range of medical conditions, with skills in developing and implementing effective treatment plans.

Preferred Experience

Preferences include experience working in assisted living or similar settings, familiarity with common geriatric syndromes, and knowledge of applicable regulations governing eldercare. Certification in Advanced Cardiac Life Support (ACLS) or Basic Life Support (BLS) is also highly desirable.
