Pulse Point Internal Medicine is seeking a dedicated and skilled Nurse Practitioner to join our full-time staff in Eastchester, New York. In this dynamic role, you will be integral to our mission of providing high-quality, value-based primary care for adults on Medicare. Your primary responsibility will be to deliver exceptional healthcare while fostering long-term relationships with patients, ensuring they receive the support needed to maintain their health and well-being.

Work Environment

This position is based in a hospital setting that fosters a collaborative culture among healthcare professionals. You will work closely with a diverse and passionate team that includes physicians, medical assistants, nurse practitioners, physician assistants, care managers, and a dedicated medical scribe. Our supportive environment allows healthcare providers to concentrate on their core missions: delivering excellent primary care and facilitating care coordination around the patient’s needs. The schedule for this full-time role is Monday through Friday, from 8:00 AM to 5:00 PM.

Key Responsibilities

As a Nurse Practitioner at Pulse Point Internal Medicine, you will undertake a range of responsibilities aimed at enhancing patient care. Your duties will include:

• Providing comprehensive and ongoing primary care for your patients, addressing routine and acute healthcare issues during office visits.
• Conducting Annual Wellness Visits and Health Risk Assessments, which require a holistic approach to health and meticulous documentation.
• Coordinating care with multidisciplinary teams, including specialists, testing facilities, and other healthcare agencies to ensure seamless care transitions and comprehensive patient management.
• Leading population health initiatives within the care team, such as ensuring eligible patients receive necessary screenings like breast cancer mammograms.
• Managing patient phone triage and outreach efforts to support active care and engagement.
• Educating patients about their health conditions, crafting individualized care plans, and discussing treatment options.
• Participating in promotional community activities to raise awareness of health and wellness.
• Conducting home visits when necessary to assess health needs in non-clinic settings.
• Reporting directly to the Medical Director while collaborating with operational leadership.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare. Our goal is to empower this demographic to live healthier, fuller lives through personalized care plans that are designed to meet their unique health challenges. As a part of our community-focused model, you will work to decrease hospitalizations by emphasizing prevention, education, and accessible healthcare delivery in patients' neighborhoods.

Qualifications

We are looking for candidates who possess the following qualifications:

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from AANP or ANCC.
• An active, non-probationary Nurse Practitioner license issued by the state of New York.
• A current DEA license to prescribe controlled substances.
• Demonstrable experience in primary care settings, preferably in internal medicine, geriatrics, or family medicine.
• A strong understanding of HCC (Hierarchical Condition Categories) documentation and ICD-10 coding.
• A collaborative spirit with a passion for teamwork and a desire to innovate within our patient care model.
• Bilingual proficiency in relevant languages is a plus.

Preferred Experience

While not required, candidates with prior experience in care coordination, population health strategies, and primary care will find this role particularly rewarding. Familiarity with electronic health record systems will also be advantageous.

Compensation and Benefits

The salary for this full-time position ranges from USD 180,000 to 200,000 annually, with consideration for experience and specialized skills. Additionally, we offer an attractive benefits package, including:
