Work Environment

This role is set within the CACC, where patient interaction occurs both in clinical settings and through house calls, ensuring comprehensive care. Key Responsibilities

As a Nurse Practitioner within the CACC, you will be responsible for:

• Obtaining detailed patient histories and conducting thorough physical examinations tailored to the cardiac population.
• Ordering and interpreting diagnostic tests such as EKGs, echocardiograms, and lab work to identify acute and chronic cardiac issues.
• Developing and managing comprehensive care plans that include lifestyle modifications, medication management, and follow-up protocols.
• Collaborating with the medical team to revise treatment plans based on ongoing patient assessments and response to therapy.
• Educating patients and their families about cardiac conditions, treatment options, and post-discharge care to empower them in their health journeys.
• Coordinating admissions and discharges in collaboration with inpatient teams to ensure seamless transitions of care.
• Documenting all patient interactions and care plans in our electronic medical record (EMR) to maintain accurate and comprehensive patient data.
• Engaging in research protocols that contribute to the advancement of cardiac care practices and integrating findings into everyday clinical work.
• Maintaining a safe, therapeutic environment and ensuring adherence to best practice standards in nursing.
• Participating in ongoing education and training to enhance skillsets and stay current with advancements in cardiology.

Patient Population / Client Focus

You will primarily serve patients with a wide range of cardiac conditions, including but not limited to heart failure, pulmonary arterial hypertension, and post-heart transplant care. Many of our patients are undergoing complex treatment regimens and advanced therapies, requiring a nuanced understanding of specialized care and an empathetic approach to support their health journeys.

Qualifications

To succeed in this role, you will need:

• A Master’s degree in Nursing with a nurse practitioner specialization focusing on adult care.
• A valid New York State Registered Professional Nurse and Nurse Practitioner licenses.
• NP board certification that demonstrates your expertise in primary and specialty care.
• Current certifications in Basic Cardiac Life Support (BCLS), Advanced Cardiac Life Support (ACLS), DEA, and NPI.
• Proficiency in electronic medical records (EMR) software, and familiarity with Microsoft Office Suite programs including Word, Excel, and PowerPoint.

Preferred Experience

We strongly prefer candidates who have:

• Two years of clinical nursing experience, particularly in a hospital setting focusing on cardiac care.
• Specific experience with patients facing advanced heart failure and those requiring complex medication regimens or mechanical support services.
• A robust knowledge base regarding outpatient therapies and interventions for cardiac health.

Compensation and Benefits
