Position Overview

Gotham Enterprises Ltd is actively seeking a skilled Nurse Practitioner (NP) to join our esteemed Center for Advanced Cardiac Care (CACC). This full-time, onsite position is based in Harrison, New York, requiring involvement in an innovative outpatient clinic that prioritizes exceptional cardiac care. You will work under the guidance of the Chief Nurse Practitioner and CACC Medical Director, delivering holistic care to our cardiac patient population.

Work Environment

This role is set within our outpatient clinic, where you will be part of a diverse and multidisciplinary team comprised of Physicians, other Nurse Practitioners, Registered Nurses, and various administrative staff. The work environment is fast-paced and dynamic, aimed at ensuring that each patient receives personalized attention and expert management of their cardiac health. You will coordinate care across different specialists and support personnel, emphasizing teamwork and communication at every juncture of the patient experience.

Key Responsibilities

• Conduct thorough patient evaluations by obtaining comprehensive histories and performing targeted physical examinations.
• Order, interpret, and utilize diagnostic tests to diagnose and manage both chronic and acute cardiac conditions effectively.
• Develop and implement individualized healthcare plans that align with established protocols, ensuring an ongoing, goal-directed approach to patient care.
• Facilitate patient education, offering guidance on disease processes and appropriate care management outside of the hospital setting.
• Coordinate with the inpatient care team to manage hospital admissions and safe discharge processes, ensuring seamless transitions in clinical care.
• Document all provider-patient interactions in our electronic medical record system, maintaining clear and compliant medical records.
• Engage in collaborative case discussions with a broad range of specialists, including Social Workers, Nutritionists, and Palliative Care teams, to deliver comprehensive care to patients.
• Participate in ongoing research initiatives and incorporate evidence-based practices into daily operations, enhancing patient care protocols.
• Maintain an empathetic, safe, and therapeutic environment for patients and their families.
• Oversee and guide the work of NP peers and nursing staff in triaging patient calls and managing communication with referring providers.

Patient Population / Client Focus

You will focus primarily on patients dealing with advanced heart conditions, including but not limited to heart failure, pulmonary arterial hypertension, and those preparing for or recovering from heart transplant procedures. Your role is pivotal in improving the quality of life and health outcomes for these patients through evidence-based care and robust patient engagement strategies.

Qualifications

• A Master’s degree in Nursing, with specialization as a Nurse Practitioner trained in adult care.
• Two or more years of clinical experience as a Nurse Practitioner in a hospital or outpatient setting, with preference for experience in Cardiology.
• Current licensure as a Registered Professional Nurse and Nurse Practitioner in New York State.
• Certification from the Nurse Practitioner Board.
• Current certifications in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), and have an active DEA and NPI number.
• Proficient in electronic medical records (EMR) systems and familiar with MS Office Suite applications like Word, PowerPoint, and Excel.

Preferred Experience

Experience managing complex patient populations, particularly those with heart failure, pulmonary arterial hypertension, and individuals undergoing advanced cardiac therapies—such as intravenous or inhaled medication therapies and mechanical circulatory support—is highly desirable. Your expertise will significantly contribute to our treatment protocols and enhance our patient care capabilities.

Compensation and Benefits

This position offers a competitive yearly salary ranging from USD 120,000 to 220,000, commensurate with experience and qualifications.
