Advanced Practice Nurse in Community Health - Full-Time, Onsite Position

Spectra Community Health is seeking a dedicated and passionate Family Nurse Practitioner (FNP) to join our team at our assisted living facility in Eastchester, New York. Work Environment

As an FNP in our assisted living facility, you will work directly with residents in a supportive and collaborative environment. Your work schedule will primarily be Monday through Friday, from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in health fairs and community outreach events may be requested during weekends, reflecting our commitment to engaging and empowering our community. An average of 40 hours per week will be expected.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, implementing personalized care plans to meet their specific health needs.
• Diagnose and manage a spectrum of acute and chronic medical conditions, emphasizing preventive care and health promotion initiatives.
• Collaborate with local organizations to design health education programs that address prevalent health issues within the community.
• Provide empathetic, culturally aware care, advocating for the rights and access to healthcare of all patients, especially those from vulnerable populations.
• Coordinate referrals and follow-up care effectively to ensure continuity across the patient care journey.
• Maintain meticulous medical records in alignment with state and federal regulations, ensuring accuracy and confidentiality.
• Engage actively in community outreach activities and health fairs, offering screenings and health education to improve awareness and access to services.
• Remain informed about community health trends, relevant legislation, and best practices, applying this knowledge to enhance care delivery.

Patient Population / Client Focus

You will serve a diverse patient population that includes individuals from various cultural backgrounds, ages, and health conditions. Your role will focus on improving the health of not only the residents within the assisted living facility but also community members who participate in outreach initiatives. By understanding and addressing the social determinants of health, you will contribute significantly to broader public health objectives.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in nursing from an accredited educational program.
• Strong interpersonal communication skills, with the ability to foster relationships with patients and community stakeholders.

Preferred Experience

• A minimum of 3 years of clinical experience, ideally in community health or public health settings.
• Experience working with underserved populations and a demonstrated commitment to health equity and social justice.
• Familiarity with electronic health record systems and other health management tools will be advantageous.

Compensation and Benefits

The competitive salary range for this position is between $180,000 and $200,000 annually, commensurate with your level of experience and qualifications.
