Work Environment

The Nurse Practitioner will work primarily in an onsite environment, engaging directly with patients in their homes and community settings through house calls. This model allows for comprehensive assessments and tailored interventions that consider each patient's surroundings and lifestyle. With typical work hours from 9:00 a.m. to 6:00 p.m., and occasional evening clinics extending to 8:00 p.m., you will enjoy a balanced yet impactful work schedule while also participating in weekend outreach efforts at health fairs and community events.

Key Responsibilities

• Conduct thorough health assessments and screenings for families and individuals, addressing their specific concerns and health goals.
• Diagnose and manage both acute and chronic health conditions, employing evidence-based practices for preventive care and health promotion.
• Collaborate closely with community organizations to design and implement health education initiatives that elevate health literacy and empower community members.
• Provide culturally sensitive care while advocating for the rights of patients, ensuring equitable access to healthcare resources.
• Coordinate referrals and follow-up care, maintaining continuity for patients through effective communication and diligent monitoring.
• Maintain meticulous medical records in compliance with regulatory standards, ensuring accurate documentation of patient interactions and treatment plans.
• Participate in community outreach programs and health fairs, utilizing these platforms as opportunities to raise awareness and offer essential health screenings.

Patient Population / Client Focus

You will serve diverse communities in the Bronx, focusing on underserved populations who may experience barriers to healthcare access. This role requires a deep understanding of the cultural nuances and social issues that affect these communities, enabling you to provide compassionate and tailored care that fosters trust and improves health outcomes.

Qualifications

• A current certification as a Nurse Practitioner and an active nursing license in the state of New York.
• A Master’s degree in nursing from an accredited program.
• A minimum of 3-5 years of clinical experience, with a specific focus on community health or public health practice.
• Excellent interpersonal and communication skills, demonstrating the ability to engage effectively with a diverse population.
• A strong commitment to social justice, health equity, and advocacy for vulnerable communities.

Preferred Experience

Experience in a primary care setting with a focus on family medicine is highly desirable. Additionally, any prior work within a concierge medicine model will be beneficial in adjusting to our practice’s unique approach.

Compensation and Benefits

We offer a competitive salary range between $120,000 and $220,000 annually, contingent on experience and qualifications.
