Work Environment

The Family Nurse Practitioner will work in an assisted living facility, providing onsite healthcare services in a warm and welcoming environment. The typical work schedule is from 9:00 a.m. to 6:00 p.m. with occasional evening clinics extending until 8:00 p.m., and participation in community health fairs or outreach events on weekends may be required. This role supports both the clinical and administrative aspects of care delivery, making for a varied and engaging workday.

Key Responsibilities

• Perform thorough health assessments and screenings, utilizing tools to evaluate both physical and mental health conditions.
• Diagnose and manage both acute and chronic healthcare issues, incorporating evidence-based practices in preventive care and health promotion.
• Engage in collaborative practices with local organizations and non-profits to develop impactful health education programs tailored to the community's needs.
• Deliver culturally aware care and serve as a dedicated advocate for patient rights and equitable access to healthcare services.
• Coordinate referrals, follow-up care, and additional needed services to ensure continuity of care for patients.
• Accurately maintain comprehensive medical records in accordance with local, state, and federal regulations.
• Participate in community outreach initiatives, including health screenings at local health fairs, to promote preventive care and community wellness.

Patient Population / Client Focus

This position primarily serves a diverse patient population, including individuals in assisted living facilities and surrounding communities in Eastchester. You will work closely with patients of all ages, focusing on holistic approaches to care that prioritize wellness, prevention, and education.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• Master’s degree in Nursing from an accredited program.
• At least four years of clinical experience in nursing, with a strong preference for backgrounds in community health or public health settings.
• Exceptional interpersonal and communication skills to effectively collaborate with patients and community stakeholders.
• A strong commitment to social justice, health equity, and advocacy for vulnerable populations.

Preferred Experience

Compensation and Benefits

• Competitive salary range of USD 180,000 – 200,000 annually, commensurate with experience and qualifications.
